Application Forms will be available on February 16, 2011 along with more information. Please check back then.

  • Forms: It is very important that the camp staff knows and understands each camper.  Therefore, parents/care providers must fill out very detailed and lengthy forms on their camper.  Camp activities are individualized for each camper, so the staff must know the camper’s communication ability, self-care skills, behaviors, interests, etc. We appreciate the time that is put into completing these forms and understand the desire to provide as much detail as possible. However, it is important to note that parents/guardians/care providers will have the opportunity to talk one-on-one with the camper’s counselor to go over any pertinent information.  After you have mailed the initial application, please fill out and mail the remaining forms (listed below).

   Failure to return forms by designated dates will result in your camper losing his/her slot and being placed at the bottom of the waiting list.

   Please consider sending in a photograph of your camper.  This may be sent along with the information form.  A photograph helps our counselors know whom they are to meet the first day of camp when their campers arrive. The picture can be returned to you when you pick up your child at camp, if so desired.

  • Fees and Refund: Every camper must pay a camp fee in advance. A deposit of $75.00 is due when the registration packet is completed and returned. The balance, $300.00, must be paid two weeks prior to  the first day of camp.  Refunds for cancellations will not be granted, unless notice is given one week prior to the camper’s scheduled arrival. The deposit is non-refundable. Those who may need assistance with the cost of camp should first contact the Indiana Autism Sertoma Club; SSHannesson@aol.com
  • Waiting List: After all of the camp slots have been filled, the remaining campers will be placed on the waiting list. The waiting list is then utilized to fill these vacancies. Campers on the waiting list will also receive the above forms. These forms must be returned for a camper to be eligible to fill a vacancy. No fee is required until the camper is selected to attend camp.
  • Early Discharge: All measures are taken to assure that each camper is appropriately served.  It is recognized, nonetheless, that there may be certain circumstances in which a camper’s behavior becomes so extreme and uncontrolled that it threatens the welfare of the entire program or poses a danger to other campers and staff. The Summer Camp Program reserves the right to discharge any camper for illness or injury, upon the advice of the Camp Nurse. In the event that early discharge is necessary, a prorated portion of the camp fee will be refunded.
  • Arrival and Departure Times: Daily arrival time for camp is 2:00 p.m. on Sunday, June 27, 2010. If a camper has not arrived by 3:00 p.m. and no notification has been sent by the camper’s care provider, then the space will be given to the first appropriate camper on the waiting list.  Departure time is 10:00 a.m. on Friday, July 2, 2010.  It is imperative that arrival and departure times are strictly observed.
  • Medication Documentation: Changes made in the camper’s medication after the medical form is signed by the camper’s physician must be documented in writing and signed by the doctor.  Otherwise, the camper will be given the medication and dosage as directed on the bottle or the medical form, whichever is most recent.
  • Miscellaneous: Please notify camp staff of any change of address, telephone number, or any other information noted on the camp forms. Parents are welcome to call camp during the week to see how things are going.
